Getting ready

Simple steps can make your visit more useful

These resources are intended to help patients and families organize information and know what to bring or discuss. They do not replace an evaluation or individualized medical advice.

Practice-approved guidance

Prepare for common types of visits and discussions

Choose the topic that best matches your upcoming appointment or question.

Prepare for your appointment

Gather the information that can help make your first or follow-up visit more productive.

  • Bring a current list of medications, supplements, and allergies.
  • Write down the concerns, changes, or questions you want to discuss.
  • Have relevant medical, school, or prior treatment information available when appropriate.
  • Contact the office before your visit if you are unsure what records are needed.

Telehealth visit guidance

Prepare a private, reliable setting for a virtual psychiatric appointment.

  • Use a quiet, private space where you can speak comfortably.
  • Check your internet connection, camera, microphone, and device battery in advance.
  • Keep your medication list and questions nearby.
  • Contact the practice if you need scheduling or connection guidance.

Medication-management visits

Helpful information to have available when discussing current psychiatric medications.

  • Know the medication name, dose, and how often it is taken.
  • Note any changes you have noticed since the previous visit.
  • Bring questions about the treatment plan to the appointment.
  • Do not change or stop prescribed medication without discussing it with the prescribing clinician.

ADHD evaluation preparation

An evaluation may consider concerns across home, school, work, and daily routines.

  • Write down the attention, organization, impulsivity, or activity concerns you have noticed.
  • Bring relevant school, work, or prior evaluation records when available.
  • Be prepared to discuss when concerns began and where they occur.
  • A psychiatric evaluation is needed before conclusions or treatment recommendations are made.

Substance-use conversations

Open, accurate information helps the clinical evaluation and treatment-planning process.

  • Share what substances are being used, how often, and any recent changes.
  • Mention other medications or health concerns that may be relevant.
  • Ask questions about the next steps recommended after evaluation.
  • For urgent medical or mental health concerns, call 911 or go to the nearest emergency department.
